BAQ Report Designer & BAQReportParameter

I would have never thought to use a Form Customization to add the
controll to feed the ReportParam fields.

What I'm actully trying to do is to creat a report that lists our
customers, with the option of listing only the customer records that
have a blank city field.

I could easily make the BAQ return only records with a blank city
field, but want to make the report a little more dynamic. This is
more of an exercise in creating a BAQ Report for others to use.
