Create new forms in vantage

About the best you can do is take a UD Entry form and hide all the native stuff and go from there

Sure using the UD01 thru 100 Entry forms. Just go to menu maintenance. New Menu item and choose the program UD01Entry etc: hang it where you want log off and on then turn on customization and design to your hearts content
