DMT Combined Cash Receipt template

Hello team,
Can anyone share a Combined Cash Receipt DMT template that can be uploaded successfully. I have been trying to upload one test record for 3 hours now without any success. First used the template with the required fields only. That one did not add a detail. I keep on adding a field I getting one error message adding another one getting a different error message. No clear cut options.

You can upload the error image and your template here to know what happens. Rule to test importing on DMT is checking the errors.

I found a very simple sample template on Epicor Knowledge Base which worked fine.
