Incidentally, the pre-printed paper we use isn&#39;t printed in-house, it&#39;s printed by the 1000 and sorted into alternating white and yellow sheets by the printing company, and for a reasonable price. For anyone in the UK, the printing company we use is Standard Group. It could be a good idea to do them in-house though, if you have the resources to make it financially viable.<br/>

Anyway, I hope my forms help, even if just to give an idea or two, since I&#39;ve picked up so many invaluable tips from the group (thanks to everyone).<br/>
<br/>
I will (hopefully) soon be adding a description of our setup onto our website, so if anyone reads it and thinks it may be very close to theirs, I would happy to share info on or off the list.<br/>
