the fiscal year and fiscal period not populated by default.

Are you double sure that all the periods for 2025 are there? These symptoms SCREAM missing Periods
period is there.
Did you select from the date picker or key that date in? The format does not look correct. At least in my screen it is a slash separator for the date does not look correct.
With that being said I see the dates on your fiscal calendar are the same… So it might be a regional setting thing on your PC…
select from date picker.

Is it possible the attempt to add update the earliest apply date was done prior to the calendar being created or periods added?
no. we did not enter the earliest apply date. it is first time.
I’m assuming you have cleared the form and attempted all steps to set the form back to an empty state:
- used the broom tool
- closed the form
- cleared the client cache
- logged out and back in again.
Attempted to add a new earliest apply date… Perhaps trying a date that matches the start date on the fiscal calendar rather than one that is in the middle of a period?
Failing that then I’m going to pull the support ticket card.
