Report builder help

We use a VB form fields in the Employee Basic Table to capture this
information. It's great for me, since I also handle the health and dental
insurance here. We don't have the payroll module, so I'm not sure if there
is a 'standard' place this data is in there somewhere?

I have tried to be good and not ask for help with Report builder. Jeremiah
left me many reports that I have been able to modify to meet the requests
give to me for reports. I now have one that has me stuck.
I have been asked to write a report that will give the name, town, state,
zip, date of birth and the type of insurance coverage (single/family...) for
each employee covered.. I can get all the info I am looking for except the
insurance coverage information. If anyone can tell me the appropriate field
that would be great. Of course if someone was to fill me in on the right
joins that might save me some time as well.

I believe you will have to set up a VB form, then extract the information
off the VB form for the type of insurance. This is what we did and it works
fine.

We use USER-DEFINED fields to reference employee
coverage-carriers-providers, etc...
