We have had this happen a number of times by our receiving clerk who is attempting to delete an RMA receipt but doesn't grab the correct context field before hitting the red X of doom. The whole RMA is just gone after that. You could activate a change log on the RMA table, but you would have to query the auditlog table as you can't exactly open up the RMA and look at the change log once it is deleted.
Eventually I am going to replace the little warning box that says, "Are you sure you want to delete this?" with something that lets the user know what they are actually deleting!
