Scrap/ Reason Code questions

We've done a little of both. For scrap codes they are number and the number is also in the description. If I recall correctly the codes are selected from a drop down list with the descriptions. The actual code means little to the operator and is mostly useful only to me when writing RB reports. We arranged the reasons in order of most frequent to least. The closest we have to "Operator Error" is "Formed Wrong" which is more likely to get used (less accusing). We do have one called "Other (Put Reason in Note)" for weird situations like "power went out during process".

We don't have rework codes (all rework is done in a new "R" job) but for RMA codes we ended up with three letter abbreviations. Again, not that the code matters to the user very much.

I've never seen it as an issue for ISO and our auditors have never mentioned anything about the coding. Main thing was saw was to keep the list short (no more than 10 reasons) and general so as to not try to divide things up too finely.

getting a list of scrap/ rework codes agreed upon here. First off, I was
wondering if most of you went with numbers (ex. 001, 002) or abbreviations
(TOOL, OPER) for your errors? Reason I ask is because of ISO 9001. Does it
make a difference?

Also, how many of you shy away from Operator Error as an error? Please let
me know. Thanks.
