SQL Conversion Error

I sorted out the problem, turns out the Unique Index scripts were never run
on this DB and this was causing the problem with the conversions which were
expecting them to have been run already.

I am getting an error on conversions on a 9.04.505C to 9.04.506.

23000: [Microsoft][SQL Native Client][SQL Server]Cannot inset duplicate key
row in object 'dbo.sysactivitylog' with unique index 'sysactivitylogprim'.

I am not sure why this is happening, I have complete permissions from the
Application Server to SQL Server which is on a different server.

There is no column for sysactivitylogprim in the dbo.sysactivitylog table,
and there are no schema changes that are required according to the Schema
change tool and everything else I can see.

Anyone have any ideas?
