Uncompleted lines on a sales order after shipping.. Vantage 6.1

It has been my experience from tracking down this exact problem in 3
different companies now, that the problem is always caused by someone
reopening the line or release.
The method to test and find the culprit is to create a BAM to log changes to
the open status of the header, line and release.
You will then find the who and when. Unfortunately, this is only moving
forward.

Also, a tip. If you close the header manually (at least in the past) it
will mark as voided. Instead if you go to the release and reopen a release
and reclose it (assuming all other release are closed) it will close the
order.

Has any one experienced seeing a sales order with only 1 line. the
sales order is shipped, and the line shows the line has been shipped.
But the line becomes a backorder somehow.
We see this when viewing a sales backlog report.
Only way we can complete the order is to physically close the order at
the sales order header.
Some complete automatically some do not, any help here from anyone.
