The BO looks like something to do with ODBC. Do you have ODBC setup? Did you try the default password option on the Custom Report Maintenance? Do you have the appserver (manager/password) and ODBC (odbcuser/odbcuser) usernames/passwords setup in the system agent? Did the report prompt for additional password after the Custom Report Maintenance screen?
