You just change your MX records so your mail is sent to them, then they sanitize it and forward it to you. Very accurate, probably catches 99% of the crap with very few false positives. Nice web interface with administrator and single user modes to configure things or examine what it's caught. It sends email twice a day to each user with an easy to scan summary of all the quarantined messages. A click on one opens it in your browser and lets you re-send it, and optionally request an exception for the sender or domain.
