The help on this provides some insight. If you don't use any custom fields, then you should not care about this. My understanding, someone correct me if I am wrong, is that if you have a custom field, and you want the data in the custom field to copy to a custom field in another table when the first table updates the second table, then you need to map the custom field to do that. For example, there is or are operation(s) that update the OrderDtl table from the DemandDetail table. If you have a custom field defined in the DemandDetail table and you want a corresponding field updated in the OrderDtl table, you need to map it in Custom Field Map.
