One other thing to keep in mind (this is probably mentioned in the 5.00 installation docs, but I don't remember, so...)
Make sure you're at the latest 3.0 patch level before you start, and after you upgrade to 4.0, you have to apply the latest 4.0 patch because one of the 4.0 patches (.815 or so, I think?) modified the database schema. Only then can you upgrade your 4.0 system to 5.0, and then onward. 5.0, 5.1, and 5.2 (so far) have not had patches that modify the schema, so it's not such an issue there.
You're in for a long weekend, Jasper, and if I could echo the good advice so many others have given here: Practice, practice practice! Don't even consider doing a gigantic conversion like this without having performed the entire procedure at least once on a separate test server with a copy of your own data. That testing will provide you with so much benefit, you wouldn't believe it. Finding out all the gotchas ahead of time, getting a baseline timing to know how long to expect each upgrade to take, etc., etc.

Thus, you will need to find the instructions for each of those steps. It
seems to me that there was information on the CD for 4.0 to get the steps to
