Hmmm... we have 3rd party hosting too but maintain our site ourselves and
upload "pages" to their server. Could you possibly develop your own form
totally seperate from your regular site content and at worst have them
publish the page? If the page was not referenced by the regular site (such
as www.oursite.com/survey.htm ) they would never have to maintain it. This
still assumes they have extensions to support automatic email return of the
results. Setting up a simple form to return results as email text is pretty
easy. No database issues, etc. Make sure they did not over quote.
But you could always do a very plain "text only" form in the body of an
email and have the recipient reply and "fill in" their answers. I would use
plain text because many people don't support HTML in email. If you wanted
to risk this you could create a something that looks like a form in Word and
save it as HTML and then place it in the email.

Todd,
That is a great idea which we wanted to do, but our site is created and
hosted by a 3rd party and they quoted us a fairly high price to do it. Do
you know of another way similar to this that wouldn't require the customer
to manually reattach the form to an email?
Thanks!

If you have a web site that supports FrontPage you could create a form on
your site that emails the results back. The web server would have to
support FrontPage extensions (or for some other development tool that
suports forms). You could create a page not linked from the rest of the
site and supply the URL direct to it in an email. You would run the risk
of
a few not having web access but most with email would likely have web
access. They could complete the form on-line and "submit" which would
take
them to a "thank you" page and a link to your regular site. A good way to
introduce them to your site as well. Web is about as universal as it gets
anymore.

Subject: [Vantage] (OT)Creating a form
Hey everyone!
We want to send out survey forms to our customers. We would like to send
them via email and create a "quick-click" type of form that they are more
apt to fill out. What do most of your companies do? I was thinking of
creating it in Excel or Access. Problem is, not all of our customers have
Access. In fact, one of our customers just now told us that he hasn't
been
able to open our Adobe emails because they don't have Adobe at their
company
(even though it's free). So, I'm not sure what type of software to create
this survey in that is universal.
Any ideas?
