Try getting into your default.mfgsys config file in your client folder.
There's a setting for "AlternateCacheFolder" By default it's blank. Make a
folder somewhere on the user's PC. like C:\Cache. Then enter C:\Cache in
that setting.
Try logging in again. I've found that this solves many "weird" problems
that are unexplainable.
I know you've tried clearing out his cache, but also give this a try. Hope
it helps.

No they will be working for awhile and then it happens. Also getting an
error that he cannot access sales order entry from time to time. Checked
personalizations and customizations and there are no warnings

You might try setting the compatibility attribute on the icon. By setting
it to 'windows 95', it should clear the error. I think you can set it back
and it will work - at least for a while. I had that issue and it would
work for months by 'toggling' that setting.
